Please indicate the status of the above project:
_____ Active, funded project, Acct # ___________
Obtain and complete the entire progress report (3
pages), sign and return it to the IACP Office (see above).
Check this if the project
has been funded, even if the animal work has not begun. Answer Sections
2 & 3 to reflect the current activity of the project. (If the animal
work has not begun, address the non-animal work that occurred during the
past year.)
_____ Funding is Pending. Keep protocol open.
-
If there are changes to the original protocol, obtain and
complete the entire progress report, sign and return it to the IACP Office
(see above).
-
If there are no changes to the original protocol, complete
SECTION 2, sign and return it to the IACP Office.
Check this if the project
has yet to be funded and you would like to keep the project open.
_____ Research activities have been completed. Project is
not going to be continued.
Close the protocol.
Complete SECTION 2, sign, and return this page to the IACP Office.
Check this if the project
has been finished and no additional work will be performed. Complete section
2.
_____ Inactive, never funded. Sign and return this page to
the IACP Office.
Check this if the project
was not funded and you do not choose to keep the project open, just in
case.
SECTION 2.
Give a brief but complete lay description
of work done and data collected during the past year.
Address the progression of the
project during the past year. Include such information as the results obtained,
articles published, presentations made, etc..
I certify that the above is a true account of the status
of this project:

SECTION 3.
1)
For each species listed below in 1)c., please provide:
a. a brief, nonscientific, description
of what is to be studied in the coming year (the questions/hypotheses being
tested, describe in lay terms):
Briefly, state the questions/hypotheses
to be examined during the coming year. This should be similar to the information
included in the progress reports submitted the funding agency, except in
nonscientific terms.
b. a brief explanation of how these studies will be conducted
(procedures, experiments, analyses, etc.):
Briefly, describe the procedures
to be performed to accomplish the hypotheses listed in 1) a. The specifics
concerning these procedures would have been included in the application
and do not need to be addressed here. For in vitro work, include the types
of analyses to be performed on the tissues collected.
c. a justification for the number of animals, per species,
requested for the coming year and complete the animal usage table below:
State the number of animals that
will be needed to perform the procedures mentioned in 1)b. and explain
how that number was determined. Include a description of experimental groups,
how the number of animals was determined (i.e. 10 million cells are needed
to yield statistically significant data for analysis "A", each animal provides
2 million cells of tissue, therefore 5 animals will be needed.)
Animal Usage
The purpose of this section to
illustrate the number of animals that where initially requested for the
duration of the project, those that have been and will be used, as well
as, the number of animals remaining on the project. This information should
be supplied for each species included in the project.

If so, please explain.
Describe any changes to the project
in complete detail, supplying the same information that is requested in
the full application.
4) Names of personnel, including principal investigator(s),
working on this project:
List all of the individuals that
will be working with the animals.
5) Location(s) where animal procedures will be performed:

the locations where the animal procedures will be performed.
6) Does the proposed study unnecessarily duplicate previous
experiments? Yes___ No___ Please justify your answer below with a written
explanation, including any databases that were searched, the date range
searched and keywords used. [USDA requirement: 9 CFR Part 2, Subpart

Answer this question, including
the most recent database searches conducted. For class B1 - D animals include
the keywords used, years searched and date the search was conducted.
