To save and use MS Word .doc files:

Choose the form that you would like to save and click it. Choose "Save File" on the next screen that appears. Now you will be able to save the file using the same method you would when using Microsoft Word or WordPerfect. Open Word or WordPerfect, then open the form that you have just saved and type your information. (Some users may need to re-save the form with the new file name once it has been opened in their word processing program.)

Once completed, submit the form as a hard copy to the IACP office. Forms will not be accepted on disk or via E-mail.
