3. Shipping charges.
4. Surcharge fee.
5. Per diem costs.

CAUTION: Please remember that NO laboratory animal, animal food, animal cage, equipment, bedding material, or other animal related commodity may be purchased or acquired from any source without prior DLAR approval.

4. Lab Animal Resources Surcharge Fee:
   1. Buyer Surcharge -
      $ 0 - 300.00 - 10% of purchase order
      $ 301 - 499.00 - 5% of purchase order
      $ 500 - over - 4% of purchase order.
   2. Overhead Charge for Supplies and Processing, other than animals-
      25% of order
