LAR Form: Request for Order of Experimental Animals

1BALANCE OF ANIMALS: This information is provided to the investigator to notify him/her of the number of animals they can purchase against their protocol.

2USDA DEFINITIONS: Information shown on the front of animal request form must correspond with information provided by the investigator to the IACUC on the application (protocol). If there is a change in the USDA Class please contact the Institutional Animal Care Program office at 7-3718.

    Class A - Animals will suffer no pain or distress greater than that produced by routine injections or venipuncture. Procedures included are injections, tattooing, blood sampling, and terminal harvesting of tissues from animals with no surgical manipulations.

    ClassB1 - Nonsurvival surgical procedures will be performed with appropriate anesthesia. Analgesics and/or tranqulizers should also be considered according to the procedure planned.

    Class B2 - Survival surgical procedures will be performed with appropriate anesthesia. Analgesics and/or tranquilizers should also be considered according to the procedure planned.

    Class C - Animals will experience pain or distress greater than that produced by routine injections or venipuncture and will not receive anesthetics, analgesics, or tranquilizers since this will adversely affect the study.

    Class D - Animals will be restrained longer than 4 hours without analgesics, anesthetics or tranquilizing agents.
