IACUC Policies - SOP Expired Protocol Policy

In accordance with federal regulations, the IACUC conducts an annual review of all studies involving laboratory animals. All protocols expire on the anniversary date of the previous approval, and must be re-approved prior to that date.
- The IACUC office sends renewal forms (progress reports) three months in advance of the due date.
- A reminder is mailed two months in advance of the due date.
- If the completed renewal form is not received in the IACP office by the close of business of the due date (or at the close of business of the next business day if the due date falls on a weekend, holiday or other day when the University is closed), the protocol will be considered "delinquent".
- Notification of a delinquent protocol will be sent to the PI with a copy to the Department Chair of the PI (to the Dean if PI is the Chair).
- If the report form is not received within ten days after the due date the PI will be sent a reminder with a copy sent to the Department Chair and appropriate Dean.
- If the report form is not received within twenty days after the due date the PI will be sent another reminder with a copy sent to the Department chair, Dean, and the Institutional Official who will inform the President of the University. (He/she should also be apprised informally of these actions as they progress through chair, dean, IO.)
- If the report has not been received by the expiration date, the protocol will be considered "expired".
- The PI will be notified by letter with a copy sent to the Department Chair, Dean, Institutional Official, Grants Management, Laboratory Animal Resources, and the President of the University.
- This letter will state that all work on this protocol must cease and that no more animals can be ordered.
- The PI must coordinate care and treatment of any animals remaining on this protocol with the veterinary staff.
- Payment of all per diem charges will remain the responsibility of the PI.
- The protocol will be presented at the next regularly scheduled IACUC meeting where a motion will be entertained for "inactivation". The PI may present information, written or in person, at the IACUC meeting to provide information relative to the decision for inactivation.
- If a committee decision is made for inactivation, the IACUC Chair will notify, through the Institutional Official, the NIH, USDA and relevant funding agencies.
Reactivation of an inactivated protocol requires IACUC action based on completion of a progress report form and a letter from the PI requesting that the protocol be reactivated. A progress report form will not be accepted any later than 6 months after expiration; a complete new protocol application will be required.
Requests for exceptions to this policy will require approval by the Chair of IACUC.
